Dr Pranav Ghody, a consultant endocrinologist and diabetologist at Wockhardt Hospitals, Mumbai Central, explains that blood sugar is influenced by more than diet alone. 'In people with insulin resistance or long-term diabetes, the liver keeps producing glucose, even when no carbohydrates are consumed. This internal production can keep blood sugar readings high, despite strict dietary control,' says Dr Ghody.
The liver normally stores glucose as glycogen and releases it when needed, such as between meals or during physical activity. However, in insulin resistance, the liver fails to respond properly to insulin's signal to stop glucose production. As a result, it continues to release glucose into the bloodstream, leading to elevated levels. This phenomenon, known as endogenous glucose production, explains why some individuals see high fasting blood sugar even after avoiding carbohydrates.
Understanding this mechanism is crucial for managing diabetes. Diet control alone may not suffice; medications that target liver glucose output, such as metformin, can help. Lifestyle factors like stress, poor sleep, and certain medications also affect blood sugar.
